NEW PEDIATRIC patient care for 8 year olds and under - Please make sure to make a SEPARATE account for each patient, and then email Dr. Miles that you’re making an appointment for a pediatric patient, and when it was booked for. She will then send you a pediatric intake, instead of the automatic adult forms that are emailed after booking a new patient. Thank you for your assistance!
